Cerebral haemorrhage is often referred to as intracerebral haemorrhage (ICH) or hemorrhagic stroke. It is a critical medical condition characterised by bleeding within the brain tissue. This condition requires immediate medical attention. This article delves into some basic knowledge about ICH to ensure proper diagnosis and treatment.
What is Cerebral Haemorrhage?
Cerebral haemorrhage or ICH, is a type of stroke that occurs when a blood vessel in the brain bursts and bleeds into the surrounding brain tissue. This bleeding disrupts the blood supply to the brain and damages brain cells.
According to research, cerebral haemorrhage is the second most common cause of stroke. Research also shows that ICH is the deadliest type of stroke, with a mortality rate of around 40%-50% within the first month.

Types of Cerebral Haemorrhage

Intraparenchymal Haemorrhage
Intraparenchymal haemorrhage (IPH) is a type of bleeding that occurs within the brain tissue. According to Wikipedia, IPH is the most common type and accounts for around 8-13% of all strokes and can arise from various underlying conditions. IPH may lead to death or significant disability This is why it is an urgent medical situation requiring immediate attention.
Intraventricular Haemorrhage
Intraventricular haemorrhage (IVH) is caused by bleeding in the ventricles, which are fluid-filled cavities in the brain. These produce and circulate cerebrospinal fluid, a protective cushion for the brain and spinal cord. It can arise from physical trauma or as a result of a hemorrhagic stroke.
According to Wikipedia, around 30% of IVH cases are primary, while 70% are secondary. It has been observed that IVH occurs in approximately 35% of moderate to severe traumatic brain injuries. Consequently, IVH typically accompanies extensive associated damage, leading to unfavourable outcomes in most cases.

What Causes Cerebral Haemorrhage

Head Trauma
Head injuries from accidents or falls can damage blood vessels in the brain. It may lead to brain haemorrhage in individuals under the age of 50.
High Blood Pressure
Untreated hypertension can weaken blood vessel walls. It increases the likelihood of a brain haemorrhage.
Aneurysm
Abnormal bulges in blood vessel walls are known as aneurysms, which occur when blood vessel walls weaken and swell. It potentially leads to a burst and subsequent brain bleeding and can result in a stroke.

Blood Vessel Abnormalities
Arteriovenous malformations (AVMs) cause weakness in blood vessels in and around the brain, often only detected when symptoms arise.
Amyloid Angiopathy
This blood irregularity is typically seen in older adults with high blood pressure. It can lead to small unnoticed bleeds that may eventually result in a larger brain haemorrhage.
Blood Disorders
Conditions like haemophilia and sickle cell anaemia contribute to lower blood platelet levels and clotting. These increase the risk of brain bleeds.
Blood Thinner
While essential for some conditions, blood thinners can increase the risk of bleeding, including cerebral haemorrhage.

Liver Disease
People with liver conditions have an elevated risk of bleeding issues, including brain haemorrhage.
Brain Tumours
Individuals with a history of brain tumours are at a heightened risk of developing cerebral haemorrhage.
Symptoms and Signs of ICH
Key symptoms of cerebral haemorrhage may include:

How to Diagnose and Treat ICH
To diagnose a brain bleed, a healthcare provider conducts an immediate physical and neurological examination, alongside reviewing the patient's medical history and symptoms. Following this, they may order imaging tests such as a CT scan, MRI, or MRA to assess the location, extent, and potential cause of the bleeding.
Additional tests, including an electroencephalogram, chest X-ray, urinalysis, complete blood count (CBC), eye exam, lumbar puncture (spinal tap), or angiography, may also be utilised to detect underlying causes.
The treatment for a brain bleed varies depending on the severity and location of the bleed. Healthcare providers strive to halt the bleeding and tackle the root cause. Treatment options may encompass surgery and medication.
Prompt medical intervention can minimise brain damage, enhancing the prospects of recovery. Additionally, patients may receive supplemental oxygen through a ventilator, nutrients and fluids intravenously or via a feeding tube, and close monitoring in an intensive care unit to ensure stable vital signs.

Mustafizur injured in BPL practice session mishap
Left-arm pacer Mustafizur Rahman sustained a head injury after being struck by a ball during practice.
The incident occurred while Mustafizur, representing Comilla Victorians in the ongoing Bangladesh Premier League (BPL), was participating in a practice session at the Zahur Ahmed Chowdhury Stadium in Chattogram on Sunday (February 18, 2024).
While returning to his run-up area during a net session, Mustafizur was hit directly on the left side of his head. He was immediately taken to a hospital in Chattogram.

Following a CT scan, the medical team from the Comilla Victorians confirmed that Mustafizur did not sustain an internal head injury. However, he will remain under observation as a precautionary measure.
“After (the) CT scan we are satisfied that he had only external injury. There was no intracranial bleeding,” stated S M Zahidul Islam Sazal, the physio of the Victorians.

Sazal further mentioned that Mustafizur received stitches for the open wound.
Mustafizur's performance in the BPL has been consistent. In nine matches, he has secured 11 wickets for the reigning BPL champions.

Shakib out of ICC World Cup 2023
Bangladesh captain, Shakib Al Hasan, is out of the ICC Men’s Cricket World Cup 2023 in India after sustaining an injury in his left index finger during yesterday's (November 06, 2023) match against Sri Lanka.
A subsequent X-ray revealed a fracture, leaving the celebrated all-rounder sidelined for Bangladesh’s final match of the tournament against Australia, set to be held in Pune on November 11.

National Team Physio Bayjedul Islam Khan provided insight into the ordeal, stating, “Shakib was struck on his left index finger early in his innings but continued to bat with supportive taping and painkillers.”
“He underwent an emergency X-ray in Delhi after the game which confirmed the fracture on the left PIP joint. Recovery is estimated at three to four weeks. He will leave for Bangladesh today to begin his rehab,” he said.
Despite the adversity, Shakib shone, scoring 82 off 65 balls and steering his team to a hard-fought three-wicket triumph against Sri Lanka in yesterday's World Cup match.

579 people killed in motorcycle accidents in 3 months: SCRF
Around 1,484 people were killed and 2,485 others injured in 1,302 road crashes across Bangladesh in last three till March 31 in 2023.
It means, 16 (16.48) people died on average per day during the period.
During the period the number of accidents involving motorcycles was the highest.
Some 579 people died in 527 motorcycle accidents which are 40.47 and 39.01 percent of total fatalities and accidents respectively, according to a report of Shipping and Communication Reporters Forum (SCRF) released on Sunday (April 09, 2023).

According to the organization, the report has been prepared based on the information published in 12 Bengali national dailies, five English national dailies, nine online news portals and news agencies and six regional dailies.
In the report, the SCRF has recommended maintaining the ban on motorcycle movement on the Padma Bridge and banning his vehicle on all highways during Eid travel to avoid accidents.
The report said that maximum 479 accidents occurred in March where 584 people lost lives and 1,102 injured.
Lowest 392 accidents resulted in 411 fatalities and 1,102 injuries respectively in February. Besides, 489 people died in 431 road accidents in January.

In these three months, maximum 421 accidents occurred on highways which is 32.33 percent of total accidents. Besides, 327 accident occurred on regional roads which is 25.11 percent.
SCRF identified 16 causes of road crashes in the report.
Unfit vehicles, unskilled and physically unfit drivers, reckless driving, overtaking, drivers and helpers’ mental exhaustion, non-fixed weekly holiday and working hours. dilapidated roads at different places significant number of risky turning points on national highways and inter-district roads, movement of slow speed three wheelers on highways, lack of capacity of BRTA and the irregularities and corruption of officials and employees concerned and poor traffic management are among those reasons.

Indian megastar Amitabh Bachchan injured while shooting film
Amitabh Bachchan said he was injured while shooting a film in southern India and is recovering at home.
The 80-year-old Bollywood superstar posted on his blog Sunday (March 05, 2023) night that he sustained a rib injury while working in Hyderabad on the science-fiction film “Project K.” It’s being made in Hindi and Telugu languages and is slated for release in 2024.
Bachchan wrote that he had broken rib cartilage and a muscle tear. He said the injury was “painful” and he consulted a doctor before he flew home to Mumbai, where he has been advised to rest.

“I shall be unable to meet the well wishers at Jalsa Gate this evening.. so do not come,” the actor said to fans who often gather outside his home in Mumbai. “All else is well,” he wrote.
He said his work on the film was suspended until he’s healed.
Bachchan has acted in more than 200 Indian films. His breakthrough came in the 1973 film “Zanjeer,” or “The Chain,” and he rose to superstardom playing bold characters, inspiring fans to copy his hairstyle, clothes and deep voice.

3 dead, 12 injured as building partially collapses in Dhaka's Science Lab area
At least three people died and twelve were injured when a building partially collapsed due to an "explosion that triggered a fire" at Dhaka's Science Lab area.
The deceased were identified as Tushar, Shafikuzzaman, and Abdul Mannan.
Tushar and Shafikuzzaman were computer operators, and Abdul Mannan was an office assistant at a company that has its office on the building’s second floor.
They succumbed to their injuries at Popular Medical College and Hospital, said Md Shahidullah, Deputy Commissioner (DC) of DMP’s Ramna Division.
Akbar, Hafiz, Asha, Jahir, and Ashraf are among the injured.
They were admitted to different hospitals in the capital, he said.
Five of the injured are receiving treatment at Dhaka Medical College and Hospital.
The three-storey building collapsed partially after the explosion at around 10:52 am.

Blast at Dhaka's Moghbazar leaves 4 injured
At least four people were injured in an explosion in Dhaka's Moghbazar area earlier today (January 24, 2023).
The blast occurred at around 9:45 am in front of a medicine store in the Moghbazar Wireless area and the injured were taken to Dhaka Medical College Hospital, said Abul Hasan, officer-in-charge of Ramna Police Station.
The injured are Saiful Islam, 36; Tareq, 20; Abul Kalam, 25; and Md Shaheen, 30.

The reason behind the explosion could not be known yet, added the OC.
"However, we are looking into whether it was a crude bomb or anything else," said Abul Hasan.
The injured are now receiving treatment at the hospital, said Inspector Bachchu Mia, in-charge of Dhaka Medical College and Hospital (DMCH) police outpost.

Woman, daughter burned in N’ganj fire
A woman and her six year old daughter sustained burn injuries in a fire that broke out at a house near Narayanganj Police Lines in the district early Sunday.
The victims were identified as Nargis Akter, wife of Monir Mia and Mariam Akter of Tagarpar area.
Dr SM Ayub Hossain, a physician of the Burn Institute of Dhaka Medical College Hospital, said the fire broke out at the house from gas pipeline leakage around 2:30 am and engulfed soon.
Nargis and her minor daughter who were at the house sustained burn injuries. Monir Mia was out of his house when the incident occurred.

Later, they were taken to a local hospital and then shifted to DMCH.
Of them, Nargis sustained 70 percent burn injuries while Mariam received 55 percent injures.

6 hurt in Awami League infighting during the party’s council in Chuadanga
Six leaders and activists of Bangladesh Chhatra League (BCL) were injured in a clash between two factions of district Awami League during its triennial council in Chuadanga on Monday.
The injured are Chuadanga district BCL’s former president Mohaimen Hasan Joardar, former general secretary Tarek Hasan, BCL leaders Feroz Joardar, Raju Ahmed, Hirak and Swapan.
A group of leaders and activists of Awami League took seats in the first row of the pandal at the council venue at Chuadanga Town Football ground this morning.
The clash broke out when some other leaders and workers led by former BCL president Mohaimen were barred from sitting in the first row.

At one stage, a group BCL activists attacked the other group including Mohaimen with chairs at the venue, leaving six people injured, said injured Hirak.
Later, the injured were sent to Chuadanga Sadar Hospital where they are undergoing treatment.
Police brought the situation under control by charging batons.
Chuadanga Police Superintendent Abdullah Al Mamun said, “I’ve come to know the matter through a video footage. I don't know who was beaten, where and when or who was admitted to the hospital. We will look into the matter.”

Patuakhali HSC examinee stabbed in front of test centre
A Higher Secondary School Certificate (HSC) examinee was stabbed by some unidentified people in front of his exam centre in Patuakhali Thursday.
Md Faisal Akon, 18, is from Hajikhali village in Madarbunia union of Patuakhali Sadar upazila and a student of Patuakhali Government College. He is taking this year's HSC examination from the Science group.
The incident took place in front of the Abdul Karim Mridha College exam centre in Patuakhali municipality around 12:45pm and Faisal was taken to Patuakhali Medical College Hospital with serious injuries.

Faisal said he went to the test centre to sit for his Biology Second Paper exam. "When I came out after the exam, five to six people forcibly took me from the gate of the centre and started beating me up as I tried to scream. At one point they stabbed me with a knife."
Although the 18-year-old said he managed to flee the scene to the college principal's room, he could not tell the reason behind the attack.
Md Moniruzzaman, officer-in-charge (OC) of Patuakhali Sadar Police Station, said: "Police visited the spot. No one has filed a complaint over the incident. We will take action if anyone files a complaint."

Acting principal of Patuakhali Abdul Karim Mridha College ANM Saifuddin said: "Foysal's hands were badly wounded in the attack that happened outside the college. He ran to my room to take shelter and was bleeding profusely."
"Foysal has an exam on Sunday. We will make an alternative arrangement, as per the rules, if he cannot write with his hands," Gazi Jafar Iqbal, secretary of the Teachers' Association of Patuakhali Government College, said. "Also, the college will take action against the attackers."
